Child Health Care Support

Child Health Care Support is an essential component of family case management plans developed by the NH Department of Health and Human Services. This program emphasizes the acquisition of parenting skills, the strengthening of parent-child relationships, budgeting, home management, communication skills, pre-vocational skill development, and supervised visitation.
Family Support Services

This program is targeted to the towns of western Hillsborough County and provides family support by mobilizing community resources needed to stabilize difficult family living situations. Specific program activities include involving the family in the pursuit of their education, employment and financial goals. The program provides information about and referral to appropriate service providers. The Family Services Support Program staff develops linkages and partnerships necessary to determine ways of supporting healthy families and promoting existing family strengths and self-sufficiency.
Manchester Academy

The Manchester Academy is a community based, yearlong, intensive and comprehensive program of punishment and rehabilitation designed to supplement or replace incarceration for felons, ideally in the first instance. Under this program the offenders are required to undertake life skills activities relating to their self-improvement plan as a condition of continued strict probation. This cost-effective method of intermediate sanction requires offenders to undertake activities that will help them become productive citizens and reduce expenditures related to prison confinement.

Manchester Community Corrections Program

The Manchester Community Corrections Program is a community based program of punishment and rehabilitation designed to serve as an alternative to incarceration of twelve months or less, on a misdemeanor charge, at the Hillsborough County House of Corrections. Following an assessment, an individualized self-improvement plan is developed to address specific needs such as life skills, education, mental health and/or substance abuse counseling, and anger management. Monitoring of this plan by the case manager and probation officer ensures compliance and addresses public safety concerns.
North Peterborough/Riverview Program

This unique program, which operates at 2 housing complexes in Peterborough, strengthens under-served families through outreach, educational workshops and recreational programs including an after-school program for children.  There are also parenting classes, seminars in decision-making, workshops in life skills, and training in job search techniques.  Staff develops linkages and partnerships necessary to support healthy families and promote existing family strengths and self-sufficiency.  Family support is also provided by mobilizing community resources needed to stabilize difficult family living situations.This program receives financial support from the Goyette Foundation.

Personal Emergency Response Systems Program (PERS)

This program provides immediate twenty-four hour access to community medical and emergency services.  The system uses of a communicator at home and a portable "HELP" button to send an electronic message to an emergency response center where trained personnel initiate emergency response procedures. In 2006 PERS served 146 eligible seniors in Hillsborough County.
Accompanied Transportation Services

This transportation program is operated under a contractual arrangement with the NH Division of Children, Youth and Family Services (DCYFS). DCYFS refers children and their family members for transport to and from case-related counseling appointments, meetings, day care, school, stores, recreation sites, residential facilities and homes of family members as ordered by the Court. Southern New Hampshire Services offers this program in the Nashua and Manchester areas.
Rural Transportation

Because many low-income, elderly, and handicapped people have limited access to transportation in rural areas of the county, we have assigned a handicapped-accessible van for use in the western area of Hillsborough County. The funding agent for this vehicle was the NH Department of Transportation. This van is stationed at our Greenville Falls elderly housing complex and provided over 12,000 passenger trips since 1997.
